The Home Health Aide is responsible for providing personal care services according to written directions prepared by the Registered Nurse (RN) and/or Therapist and carried out under the supervision of the appropriate professional.
Duties and Responsibilities:
Ability to communicate effectively while maintaining good working relationships with co-workers, managers and office staff.
Communicates to the Case Manager changes in the patient condition and needs, and/or those of the family.
Participates in interdisciplinary case conferences, in-service programs, and committee meetings.
